The Record of Student Achievement forms are designed as a potential tracking system for teachers to record the results of assessments based on the pieces that lead to the full achievement of an outcome. Teachers can record when a student has demonstrated the specific learning, or the level at which the students has achieved the learning. Teachers need to use their professional judgment to make a conclusion as to when each student has achieved an outcome.

It is important that we all assess our wellness.  The Wellness Inventory fits in with outcome W10 – understanding the 5 dimensions, what each dimension means, a conversation-starter for certain topics/issues/words/interests, and for students to gauge where their personal wellness is at the start of the semester. The information gathered from the inventory can also support the planning of students semester – the Personal Plan for Wellness.
